Attention to students enrolling in NEPHAR390 Pharmacy Practice II and NEPHAR491 Pharmacy Practice IV
For submitting the Practice folder, you need to check the submission timeline posted with this announcement.
Students needed to obey that timeline announcement in order to reduce the number of students coming for Covid-19 safety.
The place for submission is FACULTY OF PHARMACY, 3RD FLOOR, INSIDE THE CORRIDOR to the related boxes. Sign the timeline table.
Students must submit their related documents in only one folder.
The practice folder should contain;
Pharmacy Practice Application Form,
Student form (with photo),
Pharmacist’s evaluation form
Pharmacy Practice Report
Please note that the forms without photos will not be accepted.
