PHA 390 Pharmacy Practice II & PHA491 Pharmacy Practice IV  SUMMER 2022 PRACTICES

1. Students must submit his/her Pharmacy Practice Forms

• Application Form.
• Student Form with photo.
• (Pharmacist’s Evaluation Form) and Pharmacy Practice Reports to Dean’s secretariat office personally and sign the submission list during the period of October 10 to 15, 2022.

2. Pharmacy Practice Reports will be evaluated and decided for student’s success grade.
